Every person experiences Parkinson’s disease differently. Being clear and honest with your doctor will help him or her understand the impact Parkinson’s is having on you and your family. Maximize your time with your doctor by downloading copy of the "Helpful questions for your Neurologist" form below to take with you to your next visit and prioritize the topics you want to discuss. Ask a family member or friend to accompany you and to give another view of how you’re doing. Also bring a list of the medications and supplements you’re taking, with their frequency and dose.
